Document all calls and face to face visits with member and other parties. Assist and support members and family with Grievance processes and respond to any and all questions within a reasonable time. Work with Recovery and Resiliency Managers and the Behavioral team to implement intensive, enhanced care coordination when requested, which includes peer support as well as community based services designed to stabilize the member's condition, with development of WRAP plan and Crisis plan.
