These ICD-9 Codes from the Medicare HCC Risk Adjustment Model is used to determine whether these captured codes are from current or past medical history or Review of System (ROS) and flag them accordingly on the basis of supporting medical record. And verify whether each encounter is a face to face visit by an acceptable credential.• Assigned ICD9 & CPT codes to Emergency Room(ER) cases for procedures and diagnosis. • Perform focus audit and process audit in which I check others work for accuracy.• Conduct team meeting for discussing the various types of errors on a daily and weekly basis.• Train the new project joiner on client specific process guidelines.
