Review and interpret diagnostic results, ensuring timely communication and coordination of necessary follow-up care in collaboration with other Blue Door providers.≡ Team Management: lead and manage a multidisciplinary team≡ Client Care and Support: collaborate with the Blue Door Clinic Team and external providers in assessing and prioritizing client needs, ensuring timely and appropriate care or referrals.≡ Care Plan Development: lead the development and implementation of individualized care plans, ensuring comprehensive and holistic care.
