• Interview youth in foster care and perform assessments to determine client’s social, emotional, psychiatric and educational needs• Assess youth’s skills using the Ansell Casey Life Skills Assessment • Facilitate transition living planning meetings and assist with developing an independent living plan for youth aging out of foster care and transitioning to the Independent Living Unit• Maintain regular contact with youth, including regular visits with youth in their foster care home, independent living apartment, group home, residential treatment center, or college dormitory. • Maintain documentation, including D.A.P format, after all interactions with clients and enter information gathered in the OASIS software• Provide case management services • Provide advocacy, support services, counseling, and accompanying youth into community settings to meet the goals of the Independent Living Plan.• Partner with various service providers, including community-based providers of mental health care services, job training counseling, and housing • Make referrals to help youth access needed services and benefits • Develop departmental in-service trainings • Assist with the development of community-based events such as resource fairs, for youth in foster care• Observe forensic interviews of children in the Child Advocacy Center that have been victims of child abuse, neglect and maltreatment • Observe Child Protective Service (CPS) Investigations • Oversee parental visitation of youth in the foster care system• Engage in Family Assessment and Planning Team (FAPT) meetings for clients• Engage in Treatment team meetings for clients
