• Work with youth and families to develop case plans based on information collected through family engagement to support the development of family goals.• Serve as a liaison between community agencies including, local schools, social services, and juvenile domestic relations court to further support families and clients. • Administer Substance Abuse Subtle Screening Inventory (SASSI) to help identify youth’s probability of having a substance abuse disorder. Juvenile Court Case Manager Assess the needs of clients and families utilizing therapeutic interview techniques. Create case plans to address identified needs. Coordinate with the community to implement services for clients to include, but not limited to community service work, intensive in home counseling, outpatient counseling, therapeutic mentoring, and psychological assessments. Maintain accurate and complete as required by laws, district policies, and administrative regulations.

Provide structured, one-on-one behavioral health experiences for children, teens, and young adults. Coordinate activities and experiences to increase children’s participation and engagement in their community. Work with clients to develop and strengthen their communicative, interpersonal and social skills.

-Provided support to up to six adults with co-occurring developmental disabilities and mental or behavioral health needs who are temporarily accessing emergency or planned therapeutic respite-Monitor individuals for any mental health or behavioral symptoms and provide intervention and de-escalation supports in accordance with crisis guidelines in the person’s plan.-Maintains timely documentation of progress notes and other data, including incident and accident reports, behavior tracking notes, group notes, medication administration records, etc.

-Adapt classroom activities, assignments and/or materials under the direction of the supervising teacher for the purpose of supporting and reinforcing classroom objectives for classroom of four.-Monitors students during assigned periods within a variety of school environments (e.g. lunch, bus stops, playground, classroom, restroom, field trips, public transportation, assemblies, kitchen, etc.) for the purpose of maintaining a safe and positive learning environment.-Provided direct instructional services to students with learning disabilities under the supervision of a certificated staff member for the purpose of implementing IEP goals and/or remediating student deficiencies.
