-Working as a Mobile Unit, who will work year round going along the river paths, parks andstreets in and around the core downtown areas of London to actively engage people throughoutreach to apply the principles of London CAReS and its housing stability program. -The outreach team’s role is to establish a presence and a rapport with individuals who are at urgentrisk of becoming homeless and those who are chronically or persistently homeless, beingknown and trusted amongst this community. -Through relationship building, outreach workers assess the needs of a client through a standardized assessment tool, assist them to acquire housing, identify intervention recommendations through an individual service plan and support their transition to an intensive case management housing approach as part of the London CAReS model.-Application of Harm Reduction Philosophy by supplying safe and sterile harm reduction tools, as well as maintaining a safe a clean city through syringe recovery and removal as a partnership with HIV/Aids Region of London.

-Lead outreach with local schools and support services-Maintain partnership with services, businesses, local politicians and education facilities-Provide intake, assessment and case management services when needed; including referrals to other agencies where appropriate-Guide and assist clients to locate potential employment, educational opportunities or community supports-Collect and input data to track service use and outcomes-Coordinate, host and facilitate special events or workshops for employers, community partners and youth-Provide one on one and group support to clients on a variety of topics- Maintain and enhance relationships with multiple service providers that are co-located at the Centre-Develop recommendations for new initiatives and revisions to existing programs-Assist with the production and sales of Market Quality Preserves

-Coordinate monthly meetings for local community organization leaders to work together in spreading awareness of supports for abused women and men. -Record, organize and mail out meeting notes in a timely manner in order to accomplish any new business in an effective and efficient way. -Key contact for committee members and community members when planning events, meetings and sharing news to the public.
