LVN for a Skilled Nursing, Rehabilitation, and Memory Care Facility

• Greet and check-in patients and collect necessary intake information• Coordinate three provider schedules and ensure smooth office flow by communicating patient arrivals or delays• Manage patient records using EMR software while upholding required HIPPA laws for confidentiality• Verify insurance eligibility and patient dues for incoming appointments and obtain required authorizations

• Obtain patient intake of medical history & vital signs • Assist in ENT & skin related in-office sterile & clean procedures• Maintain clear and concise medical records in patient charts• Communicate with other offices and medical sites to obtain appropriate medical records• Clean and restock patient rooms; sterilize medical instruments• Complete post-op calls with patients after surgery to address any concerns and reiterate post-op care instructions
