• Leading the development and implementation of diabetes management care plans, consistent with current best practice guidelines, and a focus on prevention and management of Type 1 diabetes, Type 2 diabetes, pre-diabetes, and gestational diabetes• Facilitating ongoing nutrition education that reflects patient needs and individualized risk reduction strategies • Promoting self-management by educating patients on topics like mindful eating, carbohydrate counting, and heart health• Providing diabetes-focused health education to patients and caregivers, both individually and in group settings• Practices cultural sensitivity to clients of various ethnic, racialized, and religious minorities• Manages time effectively with a case load of 70 patients per week, daily statistics, two weekly adult education seminars, seminar curriculum development, and weekly DEP meetings • Delivering health promotion, wellness, and community diabetes outreach programs• Liaises with DEP, endocrinologists, pharmacists, and chiropodists• Supports the clinical research team by conducting clinical research studies according to the study protocol, GCP, and SOPs• Student volunteer supervisor from2017-present
