As a Registered Nurse, I would assist patients in the healing process by providing bedside care with open communication and dialogue, creating nursing care plans geared toward the individual needs of each patient, building a relationship of trust and collaboration with each patient, and working closely with other medical professionals. I earned two Daisy Awards, and took pride in the patient driven care I strove to provide.As a Nurse Case Manager, my role is to plan and coordinate care for patients from pre-admission through post-discharge by working collaboratively with the multidisciplinary team. I ensure that patients are assigned the appropriate level of care, receive the applicable services, and are transitioned to the proper post-discharge level of care. My role is to perform accurate, timely and effective discharge planning for patients.
