Provides professional, administrative and technical work in the Kansas City (MO) Health Department (KCHD). Daily tasks involve planning, coordinating, and directing resources for the purpose of facilitating HIV-related federal housing programs.

• Provided assistance to persons living with HIV/AIDS in the assessment and acquisition of needed medical and psychosocial services by the provision of case management care coordination• Held a dual position which includes a 50-client caseload and being on call daily for new intakes • Completed all intake assessments for any new persons living with HIV wanting to engage in case management services • Evaluated client’s eligibility for government funded programs, enrolled clients in comprehensive insurance plans, and provided referrals and follow up, as appropriate • Maintained regular contact with clients to provide ongoing assessment, support, and intervention • Participated on the clinic’s HIV Multi-Disciplinary Team weekly for client progress review • Provided education to clients on HIV, medication and medical appointment adherence, viral load and CD4 lab results, and risk reduction strategies

• Facilitates mandated mentorship to college age students who have received a disciplinary sanction from the Office of Student Standards and Conduct for alcohol/drug offenses • Trains in Brief and Solution Focused as well as Brief Motivational Interviewing techniques • Interns 24 hours a week conducting both group and individual mentoring sessions

• Counseled using the integrated behavioral health model/brief therapeutic interventions • Assessed patients referred by their primary care providers for behavioral health concerns• Conducted weekly psychotherapy sessions with patients under the supervision of an LCSW• Gathered community resources for patients who were uninsured using information technology
