Strive Health is a value-based kidney care company. In four years, the company has grown to over 500 employees, improved patient outcomes at lower cost, and generated over $15 million in revenue. As Officer of the Chief Medical Office, I oversaw a team that led clinical strategy, designed and implemented innovative care models that integrated a patient-centric approach. We spearheaded our health equity initiative, improving patient demographic data collection from <30% to >90%, and inspired nonclinical teams to incorporate self directed health equity learning goals in their annual OKR's. Initiating the first clinical campaign at Strive, I worked cross functionally to increase our patients optimal starts rate by 40% within a single quarter. This work helps ensure patients are optimized if they transition to ESKD and is a core competency of Strive.Using a data driven approach and patient-centric mentality, our highest performing clinical programs achieved 100% net promoter scores from patients, reduced no-show rates by 50% compared with national averages and generated over $2.4 million in savings within 4 months. Five de novo multimodality dialysis programs were launched with 0 deficiency CMS certification surveys.Most meaningful to me was the professional and personal growth demonstrated by my team.

In 2004, I started an independent private practice in nephrology and consultancy. Working with dialysis organizations, hospital systems, and risk mitigation firms, I leveraged my expertise in health care and medical network to help clients meet their clinical and business goals, among them development of care management programs, management of value-based care payment models, and implementation of technology systems to support innovative clinical programs. Results include increasing home dialysis utilization by 60% in New York, bringing together New York City fellowship programs for a city-wide home dialysis seminar, and recruiting physicians and independent physician association’s (IPA) for the Centers for Medicare and Medicaid (CMS) innovative kidney care and payment models.

As Division Chief of Nephrology, I was responsible for the academic training of residents and oversight of the quality of acute nephrology and dialysis care delivered by salaried and volunteer staff. In addition to attending the clinic and conducting research, I designed, implemented, and oversaw nephrology care management program. This included creating interdepartmental and institutional initiatives that reduced hospital associated acute kidney injury (AKI). Also while in this role I: •Noted the hospital did not have the means of capturing data necessary for quantifying outcomes. Through modernization of operations, we were able to track data for continuous quality improvement processes. •Collaborated with the NYC Department of Health in a multi-centered project accessing home blood pressure monitoring, which provided necessary information to design and implement patient-centered health care initiatives.
