Care Manager
Responsible for providing comprehensive assessment, care management and monitoring to members having a primary I/DD, which may include a co-occurring physical health, mental health, substance use disorder. • Utilizes person centered planning methods/strategies to gather information • Continually and consistently completes discovery activities (information gathering and assessment) in advance of the planning meeting• Ensures that members/legally responsible persons are informed of services available, service options available (e.g. Individual/Family Direction for Innovations participants), processes.• Assists members/legally responsible persons in choosing service providers• Facilitate referrals for clinically indicated services (e.g., I/DD services, physical health services, community• navigator to assist in social service need, mental health specialty care, substance abuse treatment)• Assists the members supported to direct the planning process/plan development• Facilitates timely development of Individual support plan, crisis plan, Health risk assessment and standards of determinants health assessments (utilizing system programs Alpha, TruCare, and NCTracks)• Monitors to ensure quality care, health/safety of the member• Closely coordinates care with the member’s I/DD providers, physical health provider(s) and, when appropriate, behavioral health providers• Monitors services for compliance with state standards, waiver requirements, and Medicaid regulations• Promotes problem-solving and goal-oriented partnership with members• Provides education and empowers members to learn and exercise rights• Connects members and caregivers to education and training to help improve member’s functioning, develop• socialization and adaptive skills, and navigate the service system• Promotes prevention and health through education on the member’s chronic conditions and/or disabilities for the member, family members, and their caregivers/support members
