Occupational Therapist
Current● Collaborate with a multidisciplinary medical team, patients family, and caregivers to form a quality plan of care for each client in my daily and weekly caseload for both Acute Rehab patients and Inpatient Acute Care patients. ● Establish appropriate client-centered short-term and long-term goals and determine barriers to goal attainment.● Provide education on goals, plan of care, and discharge recommendations for continued therapy upon return home.● Educate clients, family, caregivers, and multidisciplinary medical team as necessary regarding level of assistance the patient requires, individualized home exercise programs, home modifications, and durable medical equipment recommendations.● Perform data-analysis and task-analysis in order to re-evaluate clients' progress towards goals, revise client's plan of care if indicate, and form effective daily treatment interventions.● Document daily evaluations, progress notes, and discharge summaries demonstrating clients' progress towards goals.● Provide daily occupational therapy services to clients addressing goals to improve functional limitations.● Treat various diagnoses stroke, complex orthopedic injuries and surgeries, spinal cord injury, traumatic brain injury, multi trauma, cardiac surgeries, colorectal surgeries, cerebrovascular accidents, cognitive perceptual deficits, mental health disorders, cancer, debility, and multiple progressive diseases.
