On-Call Physical Therapist for inpatient and outpatient clinics in the greater Portland area.

Worked on the Sports Rehabilitation Team in an outpatient hospital-based orthopedic setting. Performed initial evaluations, develop plan of care, planned and led treatment sessions for patients with both surgical and non-surgical orthopedic injuries.

Worked as part of the CVA-team performing initial evaluations, developing plan of care, leading treatment sessions and facilitating discharge planning for patients with chronic and acute CVA in an inpatient rehabilitation setting. Interacted with DME vendors in order to acquire and fit all necessary DME prior to discharge. Led family training sessions for patients and their families/caregivers in order to ensure a safe transition to home. Worked closely with MD, OT, nursing and social work representative on an inter-disciplinary team to coordinate discharge planning for each patient.

Performed initial evaluation, developed plan of care and led treatment sessions addressing functional limitations in patients with surgical and non-surgical orthopedic conditions. Treated patients with a variety of diagnoses, including meniscectomy, achilles tendonitis, tibia-fibula compound fracture, back and shoulder pain.

Evaluated and developed plan of care to improve the endurance, strength, balance and mobility of geriatric patients. Encountered patients with a wide range of neurological, musculoskeletal, cardiopulmonary and integumentary diagnoses. Developed treatments and provided skilled PT in order to improve strength, ROM, bed mobility, transfers, ambulation and balance for increasing functional mobility and facilitating safe discharge to prior level of function. Worked closely with OTs, PTAs, COTAs, RNs and CNAs in order to develop the best plan of care for my patients.
