Inpatient Pharmacist- Supply medicines to ward using ‘Unit of Dose’ and ‘Unit of Use’ system.-Supply floor stock (FS), Emergency Trolley (EMT) and After Office Hour (AOH) items to ward-Intervene medication-related errors/problems detected before supplying medicines-Perform clinical screening and review of medications ordered by physicians-Carry out discharge counseling and also bedside dispensing to ensure optimum pharmaceutical care-Ensure safety and quality use of medicines through medication error reporting and routine ward check-Supervise and ensure all extemporaneous preparation, prepacking and galenical preparation are carried out following Standard Operating Procedure (SOP) and Good Manufacturing Practice (GMP)-Supply psychotropic and dangerous drugs adhering to Dangerous Drugs Act 1952-Manage inventory to ensure no under stock / over stock issue.-Ensure continuation of supply of medicines that are out of the hospital formulary by requesting from other hospital-Maintain good communication with pharmacists from other hospitals to ensure continuation of care for patients needing total parenteral nutrition (TPN) and also cytototic drugs.  Providing Warfarin Medication Therapy Adherence Clinic service according to schedule-Gave talks to nurses/pharmacy assistants to improve their knowledge on medicines
