Served Travis County residents experiencing homelessness across a Continuum of Care including local and federal agencies, nonprofit agencies, healthcare providers, and community stakeholders. Collect and analyze qualitative and quantitative data utilizing Travis County’s Homeless Management Information System and the City of Austin Municipal Court’s eCourt to maintain timely, accurate, and complete documentation. Determine accuracy and relevancy of data to inform procedural change and improvement. Develop, implement, and review individual treatment plans regularly to evaluate alternative options. Effectively communicate with management, the Court Operations division, Austin Police Department, and security staff on an ongoing basis to facilitate process improvement and ensure the safety and security of self, others, and the facility. Manage walk-in triage services by maintaining client mail and vital documents, coordinating care for Intensive Case Management clients with their assigned case manager, returning voicemails, and working as a team to see an average of 50-60 clients a day. Maintain positive working relationships with managers, supervisors, colleagues, and other stakeholders to create effective wraparound service provision and referrals. Train graduate student interns and new employees, including members of the Homeless Outreach Street Team. Assisted in the implementation of new policies, procedures, and process changes during COVID. Conduct screening and assessment to determine diagnosis and treatment of mental, emotional, and behavioral disorders, conditions, and additions including comorbid substance use and mental illness. Provide individual therapy to address motivational and psychosocial issues. Designed and implemented a pilot navigation center for individuals experiencing homelessness in Central and North Austin. Developed program evaluation protocols through a co-creation process with the City of Austin Office of Design and Delivery, social workers, and peer support specialists to capture the need for increased service provision for target population. Developed measurable goals to evaluate performance, collected and analyzed qualitative and quantitative data to determine necessary changes, established project schedule, and planned and executed two pilot locations. Provided one-on-one case management to individuals experiencing homelessness pertaining to coordinated entry, employment and housing assistance, resource navigation, and ID document obtainment. Facilitated daily meetings with staff to discuss process improvement, quality assurance, and action items.
