As the Des Moines Regional Director, I have led the team in becoming the #1 Region in thecompany for 10 of 11 months (133 programs). Patient Care Facilitators (PCT's) serve as the patient's advocate - managing the patient's overall hospital stay, coordinating communication with the entire health care team and the patient and family, and assuring that every patient has the best possible outcome.Patient Care Facilitators are the main point of contact for patients and their family. They ensure patients are engaged in their care plan by enhancing communication to ensure they are well informed and making sure the patient's needs are met in a timely manner.
