Some of my duties consist of providing direct patient care to patients, assisted with ADL’s, and administered medications to patients. Collaborate with multi-disciplinarians in reference of the patient such as physician, surgeons, physical and occupational therapy, social worker, case management, and chaplain if needed, to get the patient the correct care and supplies needed to be able to care for themselves as independent as they can in their home environment. On a daily basis I interact with patients’ families and loved ones about the progress of the patient disease process and plan of care for the patient. On this unit we work closely with cardiac thoracic surgeons and cardiologists. I am current on my ACLS/BLS. I have recently completed the Essentials of Critical Care Orientation to refresh up on my ICU skills.

Some of my duties consisted of performing weekly assessments on patients and chart observations of my assessments, collaborated with the interdisciplinary team about patient and during any change of condition of patient, and educated patients and their families on Hospice Services and what to expect in the coming days ahead, and being the patient advocate which consisted of listening to patient and families with their concerns and responding to the situation appropriately.

This is a 16 bed unit that handled a variety of medical conditions ranging from post-operative neuro-surgical procedures to emergent critical stroke patients. The unit is recognized nationally as a Stroke Center of Excellence.
