In my role as the Corporate VP for Home Care Services, I engaged in diverse responsibilities and activities across the system continuum. These include the following:Oversight of the Financial, Operational, and Quality performance of multiple hospital based home health care and hospice providers throughout the Health System.• Provided oversight and direction to 9 home health agencies in 6 states.; 3 Hospices and 1 Inpatient Hospice House in 3 states;• Implemented Telehealth program to reduce hospitalizations in all agencies • Led a team responsible for developing practices that decreased Hospitalization rates from Home Health Enterprise wide from 22% to 18%;• Brought disparate agencies together to develop standardization of process in EMR; Quality programs; clinical chronic disease management protocols and billing processes resulting in improved clinical quality outcomes; improved cash collections and standard operating procedures across agencies.• Increased Enterprise domestic capture rate from 40% to 65%.• Increased Enterprise referral rates from Physician Practices to 12% to 19%.• Implemented new technology to achieve a more robust EMR – Wound advisor, Physician Web Portal, Telephony.• Provided home health representation on all acute care clinical transformation committees • Functioned as Health System Office lead for Continuum –wide Transitions of Care Learning Community to develop and implement standard policies and practices to provide a seamless experience of care across the continuum.• Brought together representatives from across all practice settings to develop standard protocols to decrease readmissions to acute care facilities.• Facilitated alignment of acute care and post-acute practice settings to provide excellent experience of care across the continuum.
