Specialties: Geriatric Care Management: I have worked in home health care for more than 30 years. I provided hands-on skilled nursing care for several years before moving into management positions. These positions included Director of Nurses, Director of Private Duty Services, Patient Care Manager, Community Liaison, and Home Care Supervisor. For the past ten years I have focused on elder care, first through local agencies, and for the past eight years through my own private practice, Health Matters. Health Matters does geriatric assessments, development and implementation of care plans to address medical and psychosocial needs as well as life planning, communication with physicians and families, problem solving, emotional support. We also provide medication management, medical appointment coordination, referrals to appropriate services, and ongoing home safety evaluation and intervention.
