- Provide direct patient care for telemetry step down patients including, but not limited to, congestive heart failure, pre and post operative cardiac catheterization patients, and chronic kidney disease patients

- Manage specific populations with chronic conditions helping them reach their health goals and improve biometrics through health coaching, education, providing community resources, emotional support, and increasing communication with physicians- Created global bundled payment program- Manage coordination of care for orthopedic patients coming to Rush for surgery as a part of a global contract

- Provide direct patient care to 4-5 orthopedic and surgical patients- Fill in as Clinical Coordinator/charge nurse in their absence - Teacher of pre-op joint class, preparing patients for their hospital stay- Preceptor to new hire and student nurses, including education on orthopedic patients, equipment, medications, and plans of care- Chair of Quality Improvement Committee coming up with and implementing new ideas to improve patient satisfaction and quality metric

- Coordinate and set up post-hospital discharge plans for patients i.e. Rehab and Skilled Nursing Facilities or Home Health Care- Communicate with insurance companies regarding patients benefits- Provide patients and families with community resources

- Provide direct patient care with 3-5 post operative orthopedic patients including knee and hip replacements and cervical/spine fusions- Assess patients and notify physician of any critical changes- Responsible for admissions, order entry, computer documentation, and discharges- Educate patients and families on plan of care, disease process, medications, and pain control

Master'S Degree, Nurse Administration

Bachelor'S Degree, Registered Nursing/Registered Nurse
