In my current role with the Integrated Community Reablement and Assessment Service (ICRAS), I undertake emergency response and nursing clinical tasks for patients in the community. In this role, I am responsible for the nursing assessment and continuity of care of a defined number of patients, as well as managing staff working in the neighbourhood and team.As an SNP, I am one of the lead persons charged with providing proactive and responsive service that addressed the anticipatory and maintenance care needs of the patients in my defined caseload.As an SNP within ICRAS, which encompasses the Emergency Response Team (ERT) and the Intensive Community Care Team (ICCT), I undertake the holistic assessment of patient’s care needs and the provision of clinical care for patients with acute and chronic highly complex needs. In this role, I undertake clinical examination and interventions, applying evidence-based practice, local and national guidelines in clinical decision making.
