Improving Healthcare Systems For Access To Care By Underserved Patients, $2.2M Grant, PiPatient-Centered Outcomes Research Institute (Pcori ) Oct 2013 - Dec 2016Washington, Dc, UsResearch has shown that access to healthcare is a major problem and there are many other barriers, in addition to financial barriers. Despite recent changes to policy and efforts to improve efficiencies, there are still widespread problems with accessing healthcare. In this research project, The goal of our study is to examine, develop and implement patient-centered and systems strategies in seven Indiana health community health clinic systems (made up of 40 individual clinics) caring for underserved populations to improve their access to care. With this goal, our project focuses on three main objectives. First, we have identified patient-centered innovative practices and strategies to enhance access and continuity of care in CHCs through interviews with patients, providers, and clinic staff. The greater than 50 potential intervention strategies have been ranked in order using a modified Delphi approach, with experts including patients, care givers, providers, clinic staff, administrators and researchers. Second, we have employed simulation modeling to construct the revised care delivery system and validate the impact of several interventions before actual implementation. Simulation models are using EHR data (2010-2015) from each of the intervention clinics to have a sense of the variation across the clinics. Third, we are partnering with three of the CHCs to implement a multifaceted intervention that improves access to care. The intervention focuses on enhancing the culture of patient service and enhancing open access (making same day appointments available). We are focused on serving as coaches of these CHCs, in order to help them meet their goals. Expected patient outcomes are increased satisfaction, access, and engagement due to streamlined enrollment and appointment scheduling. Systems will be able to efficiently enroll patients, and prevent emergency room visits and avoid long waits at clinics. -

He also was responsible for leading the development of a comprehensive strategic plan (subsequently shared by the plan's consultant with other departments as a model), hiring several tenure track and non-tenure track faculty, a new program Director in Health Information Management, developing comprehensive policies and procedures, and developing innovative partnerships with multiple other health science and other schools for combined degrees and programs. Notably, his stewardship led to revitalization of the three programs making up the school and a strengthened financial and enrollment status for the new department. -

Business Process Management (Bpm) For Medicaid Enrollment, Scheduling And Hedis DataIndiana Primary Health Care Association Mar 2013 - Dec 2014Indianapolis, Indiana, UsAccess to health care is critical for optimal health and wellness; thus, addressing obstacles that occur at enrollment, scheduling and care delivery is important for improving the patient experience, and health outcomes. Enhanced access, continuity and quality of care are key components of patient-centered care. Notably, we identified multiple enrollment innovations implemented, which include: 1) Conducting enrollment in the community and at convenient hours; 2) Providing a multilingual financial counselor to facilitate on-line Medicaid enrollment; 3) Procedures for reviewing financial information at all visits; and 4) Facilitating resolution of incorrect provider assignment by the State. Innovations to improve scheduling included automated and live phone reminders, adding evening clinics, expanding open access, redesigning workflow to improve efficiency, alignment of providers and support personnel into dedicated teams, dashboard capabilities to display workflow, and training staff in use of the electronic IN Medicaid gateway. In summary, we have identified a number of important strategies to improve business process management. Many of these strategies have been implemented by the participating CHCs, especially with regards to effective Medicaid enrollment, scheduling and quality of care of patients in CHCs in Indiana. We have also identified several mechanisms that have the potential to dramatically improve access to care in CHCs in Indiana, by acting through policy change and informing key government stakeholders. -
