Support youth and families in obtaining permanency, maintaining stability, wellbeing, and safety. Support youth, families, and foster families to meet their highest level of functioning in all environments. Provide skills training, targeted case management and monitor progress toward permanency plan using strengths-based approaches to care. By assessing risk, development of safety plans, reporting of any abuse or neglect, or any other emergency situations. Conducting home visits per the youth’s level of supervision, communicate with providers and foster parents on a weekly basis, assess the youth and create safety plans/crisis intervention as appropriate.

Provides evidence-based individual and group clinical interventions both in the office and community settings to develop trusting relationships with clients. Engage with clients so that they may more freely discuss substance use and its effects on mental and physical health and daily functioning. Aid clients in setting their own goals for treatment, as well as plans to reduce or abstain from substance use and to learn healthy coping skills. Build rapport and a therapeutic relationship with clients to introduce them to all the elements of a healthy recovery.

Facilitate comprehensive clinical assessments by collecting data for individuals who present health or substance use and complete the required functional assessments. In conjunction with a master's prepared clinician, the Assessment Technician will help determine the individuals needs in accordance with the care plan and help identify and connect the individual to resources within the community.
