RN Case Manager for 15-20 patients. Patients are seen in their home / ALF after a new diagnosis/ an exacerbation of a current illness. Patient/ caregiver education is a very large part of this positon as well as hands on clincal treatments with skilled assessments. Aftercare of the joint replacement /general surgical patient/ medical patient are the majority of our patient population . Collaboration with PT/OT/ST/SW to coordinate patient's plan of treatment from admission to discharge is an intergral part of the job. I work closely with the patients drs and family caregivers who provide care for the patient to assure the most positive patient outcomes

Post op Ambulatory Surgery staff RN 1999-2003Med -Surg (cardiac, renal and vascular floor) 2003-2005
