Resource RN/Care Manager: assisting with Care Management and direct patient care duties in Outpatient Clinic including but not limited to central line care and maintenance, medication administration, patient education and obtaining consents for treatment. Accompany Physician and/or Midlevel provider with patient visits and assessments in Medical Oncology. Assisting Physician and Midlevel providers with follow up orders and prescriptions. Following patients for nadir and nurture calls. Following care and symptom management in between patient outpatient clinic visits. Ensuring precertification and prior authorizations complete for patient's treatment plan. Completing and maintaining Leave of Absence, Disability and FMLA paperwork for patients while receiving treatment. Outpatient clinic with six Medical Oncologists and multiple specialties including: GI, Surgery, Plastics, Pain Management, Palliative Care, Pulmonology, Thoracic Surgery, Gynecology, Urology, Orthopedics, Neurology, Neurosurgery and Psychiatry. Coordinating Care with Radiation Oncology, Rehabilitation Services, Imaging, Infusion clinic and Inpatient floor. Assigned to individual Medical Oncologist with 250 to 300 patients and specializing in GI cancers.Computer skilled and managing heavy daily patient turnover. Proficient in all documentation/record maintenance/paperwork to ensure accuracy and patient confidentiality. Certified preceptor and Mentor to new employees
