In this position I play a key role in the drafting and review of all client operations policies and procedures, and provide tailored client education regarding patient treatment plans by identifying potential care gaps and barriers. Additionally, I facilitate ongoing training and development for all newly hired employees. Furthermore, I monitor and assess care coordination plan effectiveness and adjust as needed.

During my time working in this role I delivered exceptional care to group of BPCI-A patients from pre-operative stage through ninety-day care episode, and collaborated with a cross-functional team to provide patient and family education. Furthermore, I fostered regular communication with health care providers to coordinate necessary care services. Additionally, I executed preoperative risk assessments and recommended optimal care plans to surgeons. I played a key role in the facilitation of the onboarding process for all new hires, including IHAT training, while in this position.
