Ensure youth in care are connected with outpatient psychiatric resources after discharge from inpatient hospital settings.·Investigate procedures and identify barriers for clients in obtaining psychotropic medication after being prescribed and teach other professionals the steps to ensure their clients are able to obtain their medication in a timely manner.·Facilitate monthly Child and Family Team meetings for the youth, foster parent, caseworker and professional team to ensure all the family’s needs are being met in a timely manner and model appropriate ways to facilitate clinical meetings for other professionals.·Administer direct services to licensed and relative foster parents by providing psycho-education sessions on how trauma affects the brain, understanding mental health needs, therapeutic parenting support, and PACE parenting model and Affect Self-Regulation and Competency (ARC), to help foster parents respond appropriately and keep youth with acute mental health needs in their home.·Provide family therapy sessions to youth and their foster families when needed. ·Establish brand-new documentation, timelines, child and family team meeting structure, and requirements for the pilot program while in its testing phase.
