Call Center-Making outbound calls (50-70 calls) to patients to educated them about our Chronic Care Management Program. CCM helps patients with chronic conditions such as diabetes, high blood pressure, lupus, multiple sclerosis and sleep apnea. Care Managers help patients learn to understand their conditions and live successfully with it by setting goals with the patients to help them improve there over all health. Follow up care is provided by the Care Manager as well as counseling, referrals for transportation or housing by a social worker, nutrition counseling with a dietitian and teaming up with there insurance company to learn about covered benefits or other programs covered by the insurance for the patient.
