I am responsible for data entry (MDS’s), case management, conducting initial social history interviews for new admissions and the assessing their memory and mood, discharges, having knowledge of outside services such as personal care facilities, home health care, and hospice, facilitating room changes, recording the resident council meetings minutes, attending Care Conferences, sending in maintenance request, finding/attending to residents lost items/clothes, bagging up new clothes that need labeled, and creating relations and helping with the families concerns and issues.
