Work closely with Case Managers, Social Workers and Directors of Nursing at Hospitals, Skilled Nurse Facilities, Assisted Living Facilities and Independent Living Facilities to coordinate care for patients requiring skilled services. Assess and evaluate patients for appropriateness for home health care. Obtain history and physical information from patients and care givers. Educate patients and their care givers on disease processes, medications, wound care and nutrition. Perform as a liaison between patients and physicians. Objectives to maintain a low acute care re-hospitalization rate and smoother transition for patients discharging home.

Manage referrals as they are received in the care center. Input of data into proprietary computer system. Collaborate with physician offices and hospitals to assist with initiating home health care for their patients. Obtain and write orders from physicians for various treatments and medications for their patients. Educating patients and staff on treatments, medications and disease processes. Telephone triage of patients. Verifying insurance coverage for patients prior to initiating home care. Chart audits for compliance. Maintaining and completing reports in a timely manner. Telephone follow up with patients and care givers. Intervene to resolve any customer service issues with patients, care givers, physician offices and hospitals.

Manage 7-9 RN's and LPN's. Scheduling, auditing care plans, assuring compliance with State and Federal guidelines/regulations. Oasis reviews for Medicare. Writing and monitoring orders, discharges, recertifying patients as appropriate. Initiated and oversaw cardiac telemonitoring program. Conduct team meetings, staff education, skills check off of employees. Resolving customer service issues with patients, care givers, physician offices and hospitals.

Case Manage patients in their homes. Initiating home health care and developing care plans for patients. Full physical exams, obtaining history and physical information from patients and care givers. Obtain and review all medications for drug interactions, side effects and dosing. Administer wound care, respiratory treatments, IV medications. Starting IV's, venipuncture for lab work, accessing mediports. Foley catheter insertion, removal, flushing and maintenance, suprapubic catheter insertion, removal and maintenance. Education to patients and care givers on disease process, safety, wound care and nutrition.
