Director Of Palliative Care
As Director, I provide administrative leadership, consultation, and direction to the TRUE Palliative Care program. I participate in the development and implementation of the organization's strategic plan and lead local operational plans regarding all palliative care and the hospice agency. Responsibilities as Director...1. Lead, develop and coordinate all palliative care department initiatives and programs.2. Perform all supervisory responsibilities for members of the palliative care staff.3. Complete performance evaluations for all assigned staff.4. Assess and make necessary updates to the palliative care staff organization to best benefit patient care.5. Update all palliative care staff job descriptions as needed.6. Meet regularly with palliative care staff to create departmental policies, procedures, and protocols, to educate on current trends and clinical knowledge, and to discuss operational challenges.7. Provide statistical/budgetary data to appropriate persons.8. In cooperation with the clinical management team, develop, recommend, and implement required palliative care policies and procedures.9. Develop departmental budget and manage expenses within established limits.10. Ensure compliance with all applicable state and federal laws, regulatory standards, and organizational policies and procedures.11. Ensure compliance with the Center for Medicare Services (CMS)/all stakeholders (insurance companies, hospitals, nursing facilities, etc) in relation to billing, and payment for all palliative care visits.12. Review, codes (when necessary), and authorize all palliative care related invoices.13. Participate in QAPI activities and other committees as assigned.14. Provide training for staff, medical residents and students as needed.15. Develop and implement strategies to educate the public on issues related to palliative care.
Bereavement Coordinator
Oversee the transition and grief programs within the hospice. This includes journeying with patients during the dying process and with their families up to 13 months after their death. I plan and run grief support groups. I provide grief and loss education both within our hospice, the community, and nursing home facilities. Finally, I plan the two large memorials our hospice hosts each year where we honor and pay tribute to our patients that have died within the previous 6 months.
