• Liaise with LAHSA and the County electronic outreach request and information system (LA-HOP) to deploy SPA-specific outreach requests.• Provide technical assistance to all providers utilizing LA-HOP including one-on-one training.• Assist in identifying & updating inventory/contact lists of all SPA 2 entities who are currently experiencing homelessness.• Train & inform SPA 2 Outreach teams regarding new community resources and best practices within the SPA 2 Coordinated Entry System• Cultivate relationships with supervisors from different SPA 2 agencies and communicate effectively all new outreach strategies as well as practices from the county strategy• Assist in creating, implementing, and facilitating a strategic plan on outreach response efforts to maximize new and existing resources • Prepare detailed tracking reports of outreach efforts and outcomes that result from coordinated outreach response and present outcomes to internal and external staff involved in SPA 2 CES

• Assist the most vulnerable and chronically homeless individuals targeting those clients who are vulnerable due to medical, psychiatric, or addiction needs.• Provide case management and aftercare/follow-up services to clients placed in permanent housing• Engage homeless individuals in the Skid Row area of downtown Los Angeles to provide linkages to housing and supportive services• Work in collaboration with other team members to remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits assistance, life skills acquisition• Report all statistics and updates for reporting purposes• Provide initial contact, needs assessment, information, education and referrals to the homeless• Provide case management, basic services, and advocacy for clients as well as Refer target clientele to appropriate services (e.g., health, mental health, interim housing, etc.)

• Providing administrative and research support to staff including: assisting, reviewing, and preparing literature for outreach purposes. • Sharing up-to-date information relevant to topics such as domestic violence, homicides, as well as women’s studies, and policy research. • Promote activism through prevention-based simulations, implement resources to survivors of acts such as domestic violence in high school and college settings, bring awareness to the general population and to encourage victims to report incidents of domestic violence.

• Collecting, investigating, charting and analyzing data received by interviewing patients, family members, and friends in a Mental Health setting (case management in a Psychiatric Emergency Room).• Obtaining information about the client's home environment, family relationships and investigating personality traits that may have a bearing on the individual’s medical treatment, mental health treatments and recovery all while maintaining confidentiality per HIPAA and following rules and regulations. • Reports were generated and environmental data was utilized to make recommendations concerning the individual’s treatment plan.
