Promoting and assisting care of people with acute and chronic healthcare needs. Referrals are received from primary care providers who recognize patients with a potential for failure. People are supported managing their healthcare needs primarily telephonically. The patients are guided with community resource assistance, disease education, medication management, care coordination, and social supportWorked remotely from Feb. 2022 to June 2023 in North Carolina until being replaced by Monterey.on location nurses.

Based in the Emergency Department at the Community Hospital of Monterey Peninsula (CHOMP), Transition Care Managers (TCMs) connect complex disposition patients with the correct community resources. Some of those resources are home nursing care, home infusion therapy, community group meetings, establishing PCP care, and Skilled Nursing Facility (SNF) placement directly from the ED. We receive referrals from the ED providers, who place an order in the EMR, when they have a challenging situation. Additionally we are very involved with the ED Repeat Visitor Program (ED RVP). The ED RVP identifies people who over-utilize the ED via visit frequency, notifications by insurance companies or pharmacies, and from ED staff. TCMs care manage with direct patient contact and development of a plan of care (POC). The POCs are reviewed and approved by our ED RVP physicians and become part of the patients global permanent record. This allows all ED providers to review the POC and provide care accordingly.

Managed and worked as staff in provision of patient care for 28,000 visits per years. Challenges included permanent staffing shortages heavily augmented by contract nurses and a developing health care facility. This emergency department was the ED of choice among Caribbean cruise ship tourists.Assisted in the operation and patient management of a hyperbaric chamber, treating both diving and wound care patients.
