Based in the Emergency Department at the Community Hospital of Monterey Peninsula (CHOMP), Transition Care Managers (TCMs) connect complex disposition patients with the correct community resources. Some of those resources are home nursing care, home infusion therapy, community group meetings, establishing PCP care, and Skilled Nursing Facility (SNF).
