For 13 years I did Home Visits with first time parents. Teaching them about basic infant care, doing weights and measures, developmental screenings, Referred them to community resources, brought activities to encourage partent/child intereaction, met with other agencies working with the family to coordinate services, Helped families set goal plans, organized playgroups, used curriculum to discuss various topics such as health, safety, parenting, child developmanet amoung many others. Gave parenting and helath classes. Sat on numerous commmittees in the organization (quality, employee satisfaction, grant writing, fundraising). Transported and interpreted for clients. Helped program manager with program anaylisis and plans for imrovement. Participated in giving interviews with prospective employees.For over 2 years I was a Maternal Child Health Specialist. I performed intakes with new OB patients. Including pregnancy tests, taking vitals, medical and social history as well as supporting patient with pregnancy information, community referrals and lactation support. I maintained spreadsheets and data gathering/reporting for grants. Did chart review and updated EMR throughout patients pregnancy. Followed up with patients throughout pregnancy and scheduled appointments for prenatal and postpartum care.For the last 2 years and currently I have been an Engagement and Prevention Specialist. Running reports and using that information to call patients and help them make appointments for overdue care and addressing any barriers that may have kept them from being able to come into the clinic. Reviewed and reported on quality assurance of Health Maintenance in patients charts. Interpreted for Spanish-speaking patients during provider visits. Helped patients sign up for financial assistance through local hospitals. Worked with clinical teams in order to provide thorough quality care to all patients.
