Returning Service Members,It is our duty and privilege to provide Veterans with the best available healthcare. The Birmingham VA Medical Center (BVAMC) has developed the Transition Program to assist returning Combat Veterans in connecting with their VA healthcare benefits and to help ensure a smooth transition from active duty to Veteran status. We are here to help in any way that we can.It is our goal to ensure that all eligible service members receive the highest quality care in a timely manner. Combat Veterans may be eligible for up to 5 years of cost free healthcare for conditions related to their service.Each VA medical center has a Transition & Care Management Team ready to welcome returning servicemembers and help coordinate care. The Transition and Care Management Team screens all returning combat Veterans for the need for case management services to identify Veterans who may be at risk so VA can intervene early and provide assistance.If you have served in Operation Enduring Freedom (OEF), or Operation Iraqi Freedom (OIF), or Operation New Dawn (OND), we encourage you to contact the Transition Program team.

At Aira, we are giving increased freedom and independence to individuals who are blind or visually impaired. Through a live video stream, you are able to see what they would be seeing, and provide the information they need to make decisions or explore their world.

Greater San Diego Area

Working as a case manager with active duty military personnel and their spouses in a preventive capacity; purpose of program is to ease their transition to civilian status through: brief assessment and linking to vetted community resources; also use of evidenced based practices in supportive therapy to address possible underlying issues of emotional stress.zero8hundred’s unique coalition of established veterans’ organizations ensures a streamlined one-stop approach for important services for transitioning families. zero8hundred provides access to experienced peer navigators who work closely with transitioning service members and families. Information about zero8hundred is presented during select Transition GPS classes (Goals, Plans, Success) at Navy Region Southwest and Coast Guard Sector San Diego and at career fairs at Marine Corps Base Camp Pendleton. Transitioning service members and spouses at these venues have the option of signing up for periodic interaction with peer navigators. Navigators provide ongoing support to families by proactively checking in with them from 9 months before until 12 months after separation to connect them to services and opportunities that support their transition into civilian life. zero8hundred also has a web portal available to anyone seeking transition resources that provides information on resources in the San Diego region. Implemented in January of this year, the program already has over 350 enrolled who are getting helpful personal assistance from peer Navigators!

• Develop a comprehensive strategy to create partnerships among governmental, non-profit, faith-based, and community-based organizations to enhance access, coordination, and delivery of health and human services, as well as economic and workforce development services, to veterans, military personnel, and their families. • Lead efforts that will connect veterans to job training and employment opportunities. Partner with veteran services organizations to host seminars, health fairs, job fairs, and other programs and events, which recognize the value of the military service provided by veterans, as well as connect them to services/entitlements which they earned through their military service.•Advocate on behalf of veterans and their families and connect veterans with service providers that can assist them in obtaining services and benefits such as pensions, hospitalizations, and other benefits available at Federal, State, and City agencies. •Leverage public and private resources for additional assistance to veterans and their families while they readjust to civilian life and search for education and employment opportunities.•Represent the Mayor as the liaison to governmental bodies such as the City Council, state and federal agencies, Neighborhood Councils, etc. to advise elected officials on issues affecting veterans and their families. Provide reports from veteran service organizations, as well as make recommendations to assist in the efforts to advance legislation beneficial to veterans and their families, while working to defeat legislation detrimental to them. •Prepare, examine and propose federal, state and local, legislation and/or administrative action benefiting veterans and their families.•Work with governmental, private, and non-profit organizations to develop and expand programs that address "vulnerable populations" issues such as homelessness among the veteran population, and address the needs of people with disabilities among veterans.
