Combat Veterans may be eligible for up to 5 years of cost free healthcare for conditions related to their service.Each VA medical center has a Transition & Care Management Team ready to welcome returning servicemembers and help coordinate care. The Transition and Care Management Team screens all returning combat Veterans for the need for case management services to identify Veterans who may be at risk so VA can intervene early and provide assistance.If you have served in Operation Enduring Freedom (OEF), or Operation Iraqi Freedom (OIF), or Operation New Dawn (OND), we encourage you to contact the Transition Program team.

Information about zero8hundred is presented during select Transition GPS classes (Goals, Plans, Success) at Navy Region Southwest and Coast Guard Sector San Diego and at career fairs at Marine Corps Base Camp Pendleton. Transitioning service members and spouses at these venues have the option of signing up for periodic interaction with peer navigators. Navigators provide ongoing support to families by proactively checking in with them from 9 months before until 12 months after separation to connect them to services and opportunities that support their transition into civilian life. zero8hundred also has a web portal available to anyone seeking transition resources that provides information on resources in the San Diego region. Implemented in January of this year, the program already has over 350 enrolled who are getting helpful personal assistance from peer Navigators!
