The continuing drive for excellence and improving quality is a priority for industries and service providers in the 21st Century. Safety is a highly regarding concept with zero incidents being a frequently stated goal. Safety is difficult to measure; the usual metric is the absence of events or incidents. A more effective way of improving quality is to conduct a risk analysis with the aim of avoiding, managing and mitigating the risks identifed. This approach has the capability to make the system and the individuals more resilient and therefore able to cope with variation. Creating a corporate learning environment and providing individuals with the opportunity to report ways to improve processes and mitigate risks are part of a learning culture. However these objectives are unattainable in the absence of a just culture where people are treated fairly, human performance limitaitons are acknowledged and deliberate risk takers are held accountable. My goal is to enable the development of Just Culture in service industries by translating the decades of learning that aviation has undergone over that past 30 years of Human Factors research and teaching.My experience includes: - Auditing a major human factors research project at the University of Auckland Medical School - Participating as a consumer advocate on Health Quality and Safety Commission, Safe Surgery Advisory Group- Providing human factors expertise for an Auckland District Health Board investigation- Investigating human factors aspects on a major aircraft incident and co-producing a consequential training video distributed by Boeing and Airbus- Developing human factors training packages for New South Wales Railcorp, Telecom New Zealand and aviation industry groups- Facilitating Civil Aviation industry safety forumsSpecialties: Just CultureChange ManagementHuman Factors analysis and trainingIncident investigation
