Needs AssessmentReach to Recovery Program Evaluation- Assisted with creation of evaluation proposal,timeline, and formal agreements including division of labor. Assisted with creation ofsurvey tool, recruitment of respondents, data collection, and payment schedule. Results ofthis survey were used to amend Reach to Recovery staff and volunteer training with regardsto the needs of Asian and African American breast cancer survivors and to recruit moreAsian and African-American breast cancer survivors to become Reach to Recovery volunteers.Identification of Barriers to Implementing Tell-A-Friend- Assisted with creation ofevaluation proposal, timeline, and formal agreements including division of labor andpayment schedule. Evaluation of Volunteerism and Recognition of Outcome EvaluationDesigned outcome measurement framework for breast, prostate, colorectal screeningactivities. The framework included the baseline measurement, indicator, data source, andtimeframe for measurement. As a result of outcome measurement framework, $3million wassecured for the Division and a structured, uniformed reporting system is now in use.Management Experience/Promotion of Use of Evaluation FindingsManaged an evaluation staff and contractors with an annual budget of $250,000. In twoyears, 19 contractors have been managed.Liaison to Emory University, Rollins School of Public Health, and University of NorthCarolina, Chapel Hill, School of Public Health. Maintained and built relationships withgraduate students and professors to assist the Division in conducting evaluation projects.Work with CEO, COO, Senior Vice President to identify and set evaluation priorities forthe Division. Report evaluation finds to Regional Vice Presidents and outside funders.

Collaborated with Rollins School of Public Health graduate students to conduct a variety of process, outcome evaluation using the collaborative, appreciative approach. Evaluation methods include telephone interviews, focus groups, and observations. Evaluation Reports were used as Masters Thesis and given to executive staff at American Cancer Society to inform decision-making. Final evaluations published and presented at American Evaluation Conference. Facilitated 8 member advisory board meetings where project was conceptualized. Wrote evaluation proposal, 30 to 50 question data collection tools, final evaluation reports and1 to 2 page executive summaries. Managed and supervised 5 graduate school students.

Conducted process evaluation for the State of Georgia Violence Against Women's Programthat resulted in possible increase in staff or funding; wrote evaluation proposal tosecure contract and subsequent evaluation timeline: organized and facilitated evaluationsteering committee, developed telephone interview instrument, observation tool, anddatabase, conducted phone interviews, site visits, training observations, developed logicmodels, and program delivery flow chart. Presented results of interviews to steeringcommittee. Wrote final report. Results of evaluation were used to request an increase instaff and funding

-Conducted a process evaluation on the distribution of immunization postcards to its250,000 members. As a part of the evaluation, focus groups were conducted with primarycare physicians. Created survey tools and focus group discussion guide. Designed postcardand created database with timeline as to when postcards will be mailed according to theimmunization schedule.-Began formative evaluation of the Depression disease management program.Created a needs assessment plan for Lifeguard myocardial infarction patients.Developed content for health promotion pages for the Lifeguard web site.-Medical Record Review: Coding, ICD-9, Matching codes to disease/health conditions, and office procedures.

Was a resource person for Southern California Tobacco Education Programs

Developed a five-hour training on creating tobacco education materials, which offered continuing education credits for Certified Health Education Specialists. Conducted and evaluated training in five cities throughout California.Assisted with the development and evaluation of curricula, print materials, translation, and press releases for 50 state funded tobacco control programs in southern California for Asian, American Indian, African-American, and American audience. Conducted formative evaluation of breast cancer brochures and how they are distributed toolder women who visit the Georgia Health Departments. Develop survey and focus groupdiscussion guides. Conducted focus groups that resulted in the development of three lowliteracy brochures on breast cancer diagnosis and treatment to recruit older women thathad abnormal mammograms to return to the health department for further testing.Wrote grant proposal for Susan G. Komen Foundation and received partial funding forcomplementary video about the diagnosis and treatment of breast cancer.Wrote monthly reports, which included outlining cost, materials production, anddistribution.
