• Worked collaboratively on a team of Nurse Practitioners and Registered Nurses, 86 Long Term Care Homes in Local Health Integration Network (LHIN) with a mandate of Emergency Room avoidance and capacity building.• Provided primary care to Long Term Care (LTC) residents in Hamilton and Burlington area LTC homes.• Collaborated with LTC to prevent transfers to the Emergency Department (ED), and to facilitate timely discharges from hospital.• Improved capacity building and education with LTC, to support preventative care and chronic disease management within the LTC environment.• Presented at 2017 HNHB Nurse Led Outreach Team Education Conference-Difficult Conversations.
