Senior Care Clinic

Responsible for the coordination of clinical care for patients and management of staff in a geriatric primary care office. Facilitate the growth and maintenance of the clinical program in collaboration with the Medical Director and the clinical team. Provide management for staff as well as mentoring. Additionally, triage phone calls, administer medications.

Per Diem private RN focusing on care coordination and healthcare advocacy. I work with patients and their families before, during, and after surgical procedures, interventions, or hospitlizations providing education, care, and support. Help educate and manage chronic conditions or administer acute care at home, in the hospital, or while travelling.
