Relationship building within my community, networking and forming partnerships to assist elder population. NaviCare is a SCO- Senior Care Options plan that supports individuals age 65 and older on or eligible for Mass Health Standard. Our model of care supports seniors living independently in the community with implementation of supports and services. Each member is assessed for their individual needs by our clinical care team. This program cost $0 out of pocket and has a wide selection of supplemental benefits! I love that my role makes a difference in a meaningful and impactful way-I’m all about making a difference in my community!

Educating provider accounts on our home health specialty programs. Ensuring a smooth streamlined referral process. Providing a great pt/client experience. Encompass strives to keep hospital readmission low with our VNA services in place.If you know of anyone that can benefit from home health or have any questions, please message me.

Educate community, provider groups, and community agencies on our Senior Care Options (SCO) and One Care programs. I love to network and present at community health fairs/events. Commonwealth Care Alliance (CCA) supports independence! Our programs are unique and catered by individual case by case basis. My goal is to assist eligible prospective members with our benefits and enroll them into CCA where our clinical teams will assist them and families with the best coordination of care.

Advocate for One Care members of the community who require additional Social and Psychological services.Work closely with Interdisciplinary Care Team to provide the best direction of care in a patient-centered care model and connect with community resources.Reach out to members; identify barriers and create a plan to improve quality of life enhancing indepence in community.

Collaborated with Emergency Dept Team for pt's on Emergency basis for positive outcome. Worked alongside Doctors, PA's, RN's, ED Tech's along with other Depts i.e. Radiology, Admission to assist with patient flow. Performed venipuncture blood draws, obtained vital signs, performed EKG's, assisted Providers with procedures. Worked in active CODES; performing CPR. Provided transport when required for Diagnostic testing and In-Patient admissions.
