Current- Provide nutrition education to patient's and family members on kidney health, function, and dietary needs of people with kidney disease and End Stage Kidney Disease (ESRD).
Planning nutritional meals for patients.
- Develop individualized meal plans and education related patient's medical conditions and the state of the patient's health through the nutrition care process.
- Provide continuous monitoring, examination and evaluation of patient's laboratory values and adjustments to patient's nutrition plan.
- Provide recommendations to physician regarding adjustments to nutrition-related medication based on laboratory values.
- Implemented motivational interviewing to improve patient engagement and clinical outcomes and patient adherence.
- Core team member in improving Centers for Medicare & Medicaid Services (CMS) End-Stage Renal Disease (ESRD) Quality Incentive Program (QIP) above recommend percentiles in key nutrition areas such as albumin, phosphorus, serum calcium, parathyroid hormone levels, and serum potassium.
- Certified Kidney Smart Educator, which teaches patient's diagnosed with Chronic Kidney Disease (stage 1-4) kidneys function basics, causes of kidney disease, how diet and nutrition can affect kidney disease, what health changes can be made to take control of kidney disease, why employment and the right insurance coverage are important, and which treatment options are available.
