Here at Greater Baden, I contribute to our Ryan White program. As a case manager, the client/patient is my number one priority as well as doing my part to help the under served people in our community so that they may be able to adapt to living with HIV and even transcend past what they believed possible. My responsibilities are to:*Screen HIV positive patients for Ryan White services through annual certifications.*Attain Housing, food, job and utility assistance for clients through various platforms.*Submit all patient demographics and billable services to HASTA and HRSA.*Complete psycho - social individual therapy sessions to gauge impact of COVID.*Execute 'Needs Assessment for clients in order to establish plan of action.*Moderate a monthly therapy group consisting of male patient participants,*Schedule transportation services as a means for patients to make appointments.*Attain healthcare and medication assistance for clients.*Refer clients to Dental, Nutrition and Behavioral Health services

• Conduct in-person patient intake interviews and financial screenings.• Assist HIV positive patients in attaining assistance through Ryan White, Medicaid,The exchange, MADAP/ADAP, Gilead, Viiv and Johnson & Johnson.• Obtain pertinent information from patients as it relates to their health and finances.• Prepare reports to be submitted to Ryan White/Careware in regards to patient Demographics and/ or living conditions. • Coordinate with Dept of Health in an effort to keep patients within proper care. • Follow up on pending benefit applications to ensure timely enrollment.• Assist office administrator with day to day operations as needed. • Supervise front desk receptionist.
