I provided clinical pharmacy services to the University of Vermont Medical Center neonatal critical care medical and nursing teams by prospectively monitoring medication therapies for patient-specific therapeutic endpoints and optimizing pharmacokinetic-driven individualized dosing regimens utilizing the Epic® electronic health record platform. I assisted in medication management decisions by actively participating in interdisciplinary patient-care rounds relying upon a background of critical literature evaluation and pediatric-specific therapeutic knowledge. I served as a resource for clinical drug information to the medical and nursing teams, fielding clinical questions and developing responses based upon review and critical interpretation of relevant scientific literature and clinical trial results.
