Careplan works with families, lawyers, estate planners, financial planners and other disciplines that promote wellness for older adults. Careplan provides comprehensive assessments, consultation to those being challenged with aging, and on-going care management and advocacy for older adults and their families. Careplan Geriatric Care Managers was founded by Bridget Ritossa, BA, Licensed Social Worker, Certified Geriatric Care Manager.Bridget developed a wealth of knowledge and resources in the areas of aging during her 20 years of practice working at two major hospital systems in Cleveland, as well as two of the largest nursing home chains in the country. Having her grandmother in her home while growing up helped develop a genuine love and appreciation for aging at home. Studies have shown that living at home is better for the mental health of seniors, who are happier in a familiar environment where they can live independently. Bridget's goal was to create a company that would provide older adults with the resources and knowledge to successfully age at home. Creating an individualized care plan is at the very heart of Careplan Geriatric Care Managers.
