The mission of the Baltimore City Community Action Partnership is to reduce poverty by offering opportunities for low-income households through education, financial empowerment, housing and energy services, food resources and capacity building.

The Community Assistance Network, Inc. (CAN)'s mission is to work in partnership with the community to develop, operate, and support programs that reduce vulnerability and promote personal growth, stability, and self-sufficiency among low-income residents.

Provide consultation to attorneys, complete assessment and intake of attorney referred clients. Assess individual facing criminal charges, making recommendations about mental status, serving as expert witnesses, referring defendants to community resources, and developing advocacy programs in the criminal justice system.

Assist with intake and clinical assessment of new and referred clients. Effectively provide mental health and case management services to clients. Identify and secure resources in the community to meet the needs of clients and in the absence of community resources, advocate for their development. Conduct clinical discharges and evaluations, providing referrals and arranging continued care if needed. Complete individual case notes and review individual service plans as needed ensuring client success and goal attainment. Collaborate and assist referring agencies in agency service plans

Provide resources and referrals for schooling, employment, financial management and health needs. Complete and modify as needed individual service plans ensuring clients set obtainable goals with short-term milestones. Maintain and act as a liaison between Washing D.C. Housing Authority, client’s landlords and clients ensuring all needs and request are met.Utilize Octo quick base online system to report all client interaction, issue, and concerns.

Conduct and document a minimum of four case management units per month insuring they are billed for. Document all contact and interaction with clients on internal and state database. .Provide intensive case management to clients that are enrolled in substance abuse treatment. This includes making referrals for housing, job training and other services. Encourage clients to engage in outpatient substance abuse treatment after inpatient or residential treatment has been completed

Modify and review Family service plans for clients in accordance to individual and family needs and goals.Assist families to work cooperatively with their landlords, maintain their home, maintain sobriety, seek or maintain employment and provide a nurturing environment for their children.Lead weekly support groups insuring clients remain on track with individual and sobriety goals. Completed weekly home visit and urine analyst. Upheld the mission and values of the Dayspring program insuring the programs success.Maintained a accurate record of all client interactions, and services rendered in accordance with funders policies, HIPPA Laws, and all other federal and state regulations.

Supervises youth in activities, observing or effecting changes in their social behavior. Teach rules, manners, and policies for interpersonal relationships, and personal care.Escorts children to groups, interviews, meetings, mealtime, recreation, education activities, medical appointments, and participates in supervision and instruction.Completes shift documentation including but not limited to residential case notes, incident reports, and resident inventory; also required to complete documentation requested by clinical or medical staff such as sleep logs and behavior checklists.

Develop, modify or review treatment service plans that identifies the juvenile’s needs, treatment objectives and services, behavior modification goals and release or after care planningApply various screening and assessment tools to determine the appropriate course of action and document juvenile case activities in automated system.Conduct orientation interviews with juvenile to explain terms and conditions of supervision, probation, commitment or detentionConduct juvenile group and individual behavior modification counseling sessions for juveniles
