Conduct timely in-home assessments to determine full scope of needs for ongoing caseload of clients ages 60+Develop comprehensive service plan to meet consumer choice and budget guidelinesPartner with vendors to implement in home services; monitor clients for changes to assure appropriate care Determine eligibility for public benefits and provide diligence in each application process Maintain presence in designated housing site by providing weekly office hours to be available for any resident needsCollaborate with co-Supportive Housing Case Manager to plans activities for residents promoting socialization and nutrition Assists clients and families with problem solving; makes referrals as needed to outside resourcesComply with Office of Elder Affairs and agency guidelines

-Manage caseload of 150+ low income clients who have chronic disabilities and require training for consumer directed MassHealth personal care service (Personal Care Attendant program)-Screen referrals for program via telephone to determine consumer eligibility for in-home evaluation-Evaluate consumer eligibility alongside nurse and occupational therapist by assessing individuals in person (either in-home or nursing facilities) utilizing MassHealth regulation standards -Provide skills training for client and/or family members to effectively manage self-run program -Training includes assistance on how to hire/orient a personal care worker, opening account with payroll company to pay worker, reviewing of payroll schedule/policies, etc… -Monitor client’s service by completing quarterly in-home visits and yearly re-assessments to assure proper service level -Act as a primary contact to respond and problem solve all client telephone inquiries within 2 business days -Conference with families, case managers, social workers, etc on a case-to-case basis to assure quality of consumer’s services as a whole from Bristol Elder Services -Provide referrals to supplemental services within Bristol Elders and outside sources (support groups, day centers, counseling services, etc) -Utilize numerous software programs for management of all consumer services
