Complex Continuing Care-Covered 3 steams, including:1) Dialysis: monitored patients electrolytes and extended electrolytes, and provide recommendations for adjusting phosphate binders, dialysate baths, and supplemental/dietary adjustments2) Medically Complex: tailored patients enteral and parental nutrition care plans based on anthropometrics, biochemical results, and medical status.
