• Implemented evidence-based clinical practice transformation best practices for 300 primary care and specialty care practices, engaged in TCPI in Texas, Oklahoma, Arkansas, Missouri, and Puerto Rico.• Delivered 1200 CMS physician practice assessments, up to 30 assessments simultaneously.• During the course of the assessments, provided technical assistance to practices on how to embed quality improvement methodologies, policies and procedures to hardwire the quality improvement, and how to positively impact practice cultures.• Supported clinicians in achieving large-scale practice transformation through the development of quality improvement strategies, reducing inefficiencies and costs.• Educated practices on components of care management, care coordination and developing patient-centered medical homes.• Coached practices in quality payment reform in the CMS Quality Payment Program.• Cultivated partnerships with seven Practice Transformation Networks across Texas, Oklahoma, Arkansas, Missouri, and Puerto Rico to support their practices in transformation and assessment completion. • Supported the achievement of surpassing contract goals for total number of assessments completed at 140% of base goal and 70% of stretch goal. • Produced two CMS program analysis publications: • Primary author, CMS 11th SOW Quality Improvement Network-Quality Improvement Organization C.3 Care Coordination State of Oklahoma SQUIRE titled “Transitional Care Management Clinic – A Team Approach to Reducing Readmissions in an Academic Family Medicine Setting.”• Co-author on the TMF Health Quality Institute’s CMS Chronic Kidney Disease (CKD) Special Innovation Project (SIP) for Texas, Oklahoma, Arkansas, Missouri, and Puerto Rico SQUIRE titled “The Final Report on Improving Early Diagnosis and Treatment of Chronic Kidney Disease (CKD) via an Integrated, Systemic Approach Targeting At-Risk Medicare Fee-For-Service Beneficiaries with Diabetes and/or Hypertension.”

• Managed the RMHP Regional Learning Faculty contract, communications and monthly webinars, collaborations with other regional and national organizations, reports, and eight conferences for successful implementation of the CMS Comprehensive Primary Care Initiative (CPCI) serving 14 practices.• Facilitated and trained 25 primary care (primary care, internal medicine and pediatric) practices in practice transformation and quality improvement curriculums to increase efficiencies and decrease waste, contain and decrease costs, utilize data to drive quality improvement efforts, and improve patient safety and satisfaction in meeting the Quadruple Aim.• Created, presented and successfully delivered 26 additional Learning Collaboratives and conferences, for clinicians and all levels of staff of primary care practices, community organizations and hospitals across Colorado over three years.• Managed the collaboration contract with the Informed Medical Decisions Foundation (IMDF) to deliver a four part-webinar series, instructed practices on how to implement shared decision making by utilizing motivational interviewing communication techniques.• Distributed 200 IMDF decision aids and resources to engage patients, positively impacting quality of patient care and patient experience.• Supervised the MOC tracking and Performance Improvement (PI) CME and Part I AMA PRA Category 1 CME credit programs and reporting for the Quality Improvement and Clinical Program Development and Evaluation departments at RMHP.• Successfully coordinated and wrote Maintenance of Certification (MOC) Part IV: Quality Improvement applications to the American Boards of Family Medicine (ABFM - 20 points), Internal Medicine (ABIM - 20 points) and Pediatrics (ABP - 25 points) for the Colorado Beacon Consortium curriculum.
