Care coordinators work through guidance and collaboration with the network providers to provide mental health services for youth and families assigned by Milwaukee County Children's Court Officials.Weekly home visitation for youth and families place both in home and out of the home by Milwaukee County Children's Court Officials and the Milwaukee County Bureau of Child Welfare.Crisis prevention and intervention for youth and families participating in Milwaukee Wraparound.Provide transportation for youth and families for appointments, including but not limited to: court hearings, probation, mental health and physical health appointments, etc.Work with formal and informal supports to prevent the youth from reoffending.Hold youth accountable for schooling and appointments through daily visitations and records requests.Enter information into the county's synthesis system.Prepare daily contact notes using the county's synthesis system.Create crisis prevention plans and progress reports through Plans of Care.Write and submit court reports to Milwaukee County Children's Court depicting progress and concern of youth's community and individual development.Work with at-risk youth including sex offenders and violent criminals to prevent reoffending and create treatment plans.Work to reunite youth and families by working in collaboration with juvenile probation, mental health providers, Bureau of Child Welfare, Public Defender's Office and other court officials.
